This form needs to be completed if you are requesting permission to use non-organic seed, agricultural ingredients, food additives or processing aids.

This form needs to be completed by organic producers if GM varieties are available in Canada for the conventional seed approved for planting. For example, there are genetically modified corn hibreds and soybeans varieties being grown in Canada.

This form also needs to be completed by processors if non-organic agricultural ingredients, food additives or processing aids are being used and there is a possibility that GM or GE materials may have been used in producing the ingredient, food additive or processing aid.

This form needs to be completed and include fields of all organic status, organic, transitional and conventional.
